		<description>Quick addition to the Medical Authorization form - I also added our children&#039;s blood types, current weights, and any regular medication with dosage, as well as all of our doctor, dentist, pharmacy and preferred hospital information to this form for family members who care for our kids occasionally.  In an emergency, our kids would be able to get quick medical/dental help, and that is an important piece of mind for me.  Thanks!</description>

I have one more quick suggestion to put up at the top of your &quot;grab and go&quot; list.  A black sharpie marker. If you have to evacuate take a minute to write the following on the back of your child(ren): name, cell phone number, out of town number (grandparents?), and allergies/meds. I hate to think of it but there is a chance members of the family might get separated and this info would be very helpful for anyone who found your child(ren). Even older kids might get nervous in an emergency situation and forget.</description>
